• Job Requirements
• Must be AHA BLS Certified
• Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals
• Nursys quick confirm verifications should be completed for all APRN, RNs and LPN/LVNs
• Completed Ascension Color Vision requirement prior to start and updated every 3 years
• HIPAA, Joint Commission -NPSG, OSHA Topics competency required
• Ascension EKG Exam with score needs to be 90% or higher with only 2 attempts
• Ascension Onboarding Requirements Education Attestation completed
• Ascension Nursing Contract Worker Onboarding done onsite
• Ascension MOAB certification required
• Ascension Nova StatStrip training required
• Vaccination requirements for MMR
• Annual Fit Test required
• Hepatitis B documentation of vaccination series completion or declination
• Influenza Vaccine required unless in exempted category
• Complete a Statement of good health or physical exam prior to first assignment, updated annually
• Driver's License/State ID compliance for compact license if working in other than home state
• 2 Step PPD OR IGRA (Quantiferon/Tspot) required
• Documented proof of Varicella immunity or vaccination
• Tdap vaccine valid for 10 years required
• Ascension Profile and 2 Professional References for submission
• Thorough Ascension Criminal Background Check Results
• Annual Ascension Skills Checklist completion in specialty
• Ascension 12 Panel Drug Screen requirement met
• Headshot for Ascension Badge Photo with a white or blue background
• Ascension Education Verification at source level
• Ascension Confirmation of Assignment uploaded
• Ascension Influenza Vaccine Exemption/Declination policy
• Completion of Ascension Exit Survey at the end of assignment
